Residual Risk of Undetected Infection Estimator – A Decision Support Tool for Transplant Clinicians

This specialised online tool supports donation and transplantation professionals by generating a tailored Residual Risk Report for individual organ donors. It calculates the estimated risk of undetected recent infection with HIV, Hepatitis B, or Hepatitis C, in cases where standard serology and NAT screening results are non-reactive.

Designed for use at the point of clinical decision-making, the tool:

  • Provides quantitative estimates of residual viral risk based on donor history and testing data
  • Integrates evidence from peer-reviewed epidemiological studies, including behavioural risk group incidence rates
  • Factors in critical care duration, viral replication dynamics, and NAT assay sensitivity
  • Produces a clinician-facing risk report and an accompanying recipient handout to support informed discussions with transplant candidates

Built on a rigorous evidence base, this tool helps clinicians weigh the benefits and risks of using organs from donors with increased viral risk behaviours—supporting both safety and organ utilisation.
